Below are the registration forms for both Segment 1 and Segment 2 courses. If you are 18yrs + and are interested in Behind-The-Wheel (BTW) driving, please complete the BTW Driving Adult Contract.

Please complete and return your registration form to the instructor prior to the start of your course.
